It includes the Directed Speaker Dock (missing the little plate that goes under the radio - cosmetic), and car mount, 2 antennas, a wall power supply, a car power supply and a remote (may need a new battery).
I have had this radio for more than a year. I purchase these second hand. They usually update monthly or after they have been in storage for a while - like this one. It is shown updating in one of the pics.
Whenever I find these active radios I hold on to them and use them for at least 7-8 months to a year to verify that they are indeed going to keep playing and have lifetime subscriptions. I had this one in storage for more than a year and it just updated today. I have more that I will be posting in coming weeks.
XM and Sirius both offered Lifetime subscriptions before they merged. I have found that only certain radio models from either are likely to have these subscriptions.

Although the these are active with a lifetime subscription, the radio is an electronic item and by no means should one consider the device a lifetime unit.
The music will play as long as the radio lasts. Take care of it.

The radio SID (serial number) has been photographed. So, please don't try to swap the radio.
